We are an 84-bed Skilled Nursing/Short Term Rehabilitation facility located in Wellesley Hills, MA, dedicated to providing exceptional rehabilitation and nursing services to our patients. Summary We are seeking an experienced, licensed Physical Therapist to join our team in a per diem role. Looking for someone who is able to cover shifts on weekdays and weekends. Responsibilities Conduct comprehensive assessments of patients' physical abilities and needs. Develop and implement individualized treatment plans based on patient goals. Provide hands-on physiotherapy treatments and modalities. Educate patients and their families on rehabilitation processes and home exercises. Collaborate with interdisciplinary teams to optimize patient care. Monitor patient progress and adjust treatment plans as necessary. Maintain accurate documentation in EMR systems. Participate in discharge planning to ensure continuity of care. Qualifications Physical Therapy degree from an accredited program. Current MA licensure as a Physical Therapist. Experience in acute care or skilled nursing settings preferred. Excellent communication and interpersonal skills. Ability to work collaboratively within a team environment. Familiarity with EMR systems is a plus (specifically PCC).

From $55.00 per hour
