Per Diem Physical Therapist (PT) - Home Health Visits $100 Per Visit | Flexible Schedule Read this first (important): This is a per diem (as-needed) role — not full-time and not a guaranteed schedule. We are building our Physical Therapy service line and seeking a licensed Physical Therapist to perform home health visits throughout Milford, Framingham, Worcester County, and surrounding communities.

Your Role:

Provide skilled Physical Therapy services in patients' homes. ⭐ Perform initial evaluations ⭐ Complete reassessments and discharge visits ⭐ Develop individualized treatment plans ⭐ Educate patients and families on mobility, safety, and home exercise programs ⭐ Collaborate with nurses, physicians, caregivers, and the care team ⭐ Complete documentation accurately and on time

Who We're Looking For:

⭐ Active MA Physical Therapist License ⭐ Minimum 1 year Physical Therapy experience ⭐ Home Health experience preferred but not required ⭐ Valid driver's license + insured car (policy can be under another name) ⭐ CPR/BLS (American Heart Association or Red Cross only ) — can be renewed during onboarding if expired ⭐ TB Test & Physical (within 1 year) — can be completed during onboarding ⭐ Strong assessment, communication, and documentation skills
