We are currently seeking a compassionate, motivated, and experienced Physical Therapist (PT) / Occupational Therapist (OT) to join our growing team. This is an excellent opportunity for an Physical Therapist (PT) / Occupational Therapist who is passionate about helping clients regain function, improve their quality of life, and maintain independence through personalized therapy interventions. Responsibilities Conduct comprehensive assessments to evaluate clients' functional abilities, limitations, and therapy needs Develop and implement individualized treatment plans in collaboration with physicians and other healthcare professionals Assess home environments and recommend modifications to improve safety, reduce fall risks, and enhance independence Identify adaptive equipment needs and provide recommendations to maximize client function and quality of life Monitor client progress, adjust treatment plans as needed, and communicate changes in condition to the Director of Nursing and physicians Coordinate care with interdisciplinary team members, caregivers, family members, and community resources to ensure continuity of care Educate clients, families, and caregivers on therapeutic interventions, adaptive techniques, and safety strategies Prepare accurate clinical documentation, progress reports, treatment summaries, and discharge plans in a timely manner Maintain compliance with all professional, regulatory, and agency standards while delivering exceptional patient-centered care Qualifications Current MD Occupational Therapist (OT) or Physical Therapist license required Minimum of one (1) year of recent experience as an Occupational Therapist or Physical Therapist Strong clinical assessment and treatment planning skills Ability to work independently and effectively manage time and caseloads Excellent communication, interpersonal, organizational, and problem-solving abilities Experience with electronic medical records (EMR) preferred Passion for providing compassionate, high-quality home health care Why Join BrightStar Care? $85.00 - $100.00 per hour
