Position Summary:
The Physical Therapist evaluates, plans, and provides physical therapy services to residents in accordance with physician orders and individualized care plans. The Physical Therapist promotes mobility, independence, safety, and quality of life while supporting rehabilitation and restorative goals. Essential Functions Evaluate residents and develop individualized treatment plans. Provide physical therapy treatments and interventions to improve mobility, strength, balance, endurance, and functional independence. Establish measurable goals and modify treatment plans based on resident progress. Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility, state, and federal requirements. Communicate resident progress and recommendations to physicians, residents, families, and the interdisciplinary team. Supervise Physical Therapist Assistants and support personnel in accordance with applicable regulations. Participate in care planning meetings and quality improvement activities. Maintain therapy equipment and treatment areas in a safe and organized manner. Comply with all facility policies, professional standards, and regulatory requirements. Qualifications Graduate of an accredited Physical Therapy program with current state licensure. Experience in skilled nursing, long-term care, or rehabilitation settings preferred. Strong communication, documentation, and clinical assessment skills required. Ability to stand, walk, bend, reach, lift, transfer, and assist residents throughout the workday.
