Job Summary We are seeking an experienced Physical Therapist (PT) for a Home Health contract opportunity in Battle Creek, MI. The therapist will provide comprehensive physical therapy services to patients in their homes, helping improve mobility, strength, function, and independence. Home Health experience is required.

Schedule:

Monday-Friday, 8:00 AM-5:00 PM | 40 hours/week

Weekend:

Rotation every 4-6 weeks Requirements Active Michigan PT license Minimum 1 year of experience Home Health experience required AHA BLS certification Valid driver's license, reliable vehicle, and auto insurance Supervisor-level references required Responsibilities Evaluate patients and develop individualized treatment plans. Provide physical therapy in patients' homes. Monitor progress and adjust treatment plans as needed. Educate patients and families on exercises and care plans. Maintain accurate and timely documentation.

Pay:

$55.00 - $65.00 per hour
