Job Description

Optimal Care is seeking a compassionate Home Health Licensed Physical Therapist to provide one-on-one rehab services in patients' homes. In this role, you will complete comprehensive evaluations, design individualized treatment plans, and deliver evidence-based interventions to restore mobility, strength, and independence. You will educate patients and caregivers, ensure accurate EMR documentation, and collaborate closely with an interdisciplinary team. This position offers autonomy, schedule flexibility, and the opportunity to build meaningful, long-term patient relationships in the community. Responsibilities Perform comprehensive in-home PT evaluations and reassessments Develop individualized, evidence-based treatment plans and goals Provide therapeutic exercises, gait training, and balance interventions Educate patients and caregivers on home exercise programs and safety Document all visits and progress notes in EMR accurately and timely Collaborate with nurses, physicians, and other disciplines on care plans Monitor patient progress and adjust interventions as needed Ensure compliance with all regulatory and agency policies Participate in case conferences and interdisciplinary meetings Provide fall risk assessments and home safety recommendations Required Skills Physical therapy evaluation and treatment planning Home health care practices and regulations Gait training and balance rehabilitation Orthopedic and neurologic rehabilitation techniques Pain management modalities Patient and caregiver education EMR and clinical documentation Home safety and fall risk assessment Functional mobility and ADL training Interdisciplinary care coordination
