Physical Therapist - Full-Time | Bronson at Home (Home Care) Sign-On Bonus Up to 10,000 dollars Bronson at Home is seeking a compassionate, skilled Physical Therapist to join our Home Care team. In this role, you'll deliver personalized, one-on-one therapy in patients' homes, helping them regain mobility, manage pain, and improve daily function. What You'll Do Complete comprehensive assessments of mobility, strength, balance, coordination, and functional abilities Develop individualized treatment plans and measurable goals Provide evidence-based physical therapy interventions in the home setting Educate patients, families, and caregivers on exercises, safety, and self-management Collaborate with physicians, nurses, and the interdisciplinary care team Recommend and train patients on assistive devices and adaptive equipment Support quality improvement and serve as a clinical resource within the rehab team Required Qualifications Graduate of an accredited Physical Therapy program Current Michigan Physical Therapist license Michigan driver's license, reliable transportation, and auto insurance BLS certification within three months of hire Strong communication and interpersonal skills Basic computer and mobile device proficiency Preferred Qualifications Previous physical therapy experience Home care experience Why You'll Love This Role Sign-on bonus up to 10,000 dollars Meaningful one-on-one patient care Independence and flexibility in managing your schedule Ability to see patient progress in their home environment Supportive, collaborative rehabilitation team Key Responsibilities Clinical Care Interpret physician referrals and establish functional baselines Deliver therapeutic activities and exercise programs Provide patient, family, and community education Assist with goal setting and treatment duration Design or recommend assistive devices Provide coverage across Rehabilitation Services as needed Documentation & Administration Complete timely, accurate documentation in the electronic health record Maintain confidential and compliant patient records Communicate documentation barriers to leadership Participate in committees, task forces, and process improvement Demonstrate strong organization and time-management skills Physical Requirements This role requires frequent standing, walking, bending, and assisting with patient mobility. Candidates must be able to lift and move light objects and safely support patient transfers and positioning.
