Manage discharge planning processes to ensure seamless transitions from inpatient to outpatient or home health settings. Collaborate closely with healthcare professionals including physicians, nurses, occupational therapists, and case managers to optimize patient outcomes. Requirements Valid license or certification to practice as a Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Demonstrated ability to develop effective care plans for diverse patient populations including geriatrics, pediatrics, orthopedics, neurology, and post-surgical cases. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation purposes. Acute care experience preferred; familiarity with inpatient hospital procedures is advantageous.
