  • PRN Physical Therapist
  • to provide care to our patients in our Midland, MI branch.
  • What You'll Do as a
Home Health Physical Therapist:
  • + Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
+ Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function. + Adjust treatment plans and techniques as needed to achieve optimal outcomes. + Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care. + Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices. + Document all visits, assessments, and progress notes accurately and in a timely manner. + Support patients in maximizing their independence and improving quality of life—wherever they call home.
  • About You
  • Qualifications -
What You'll Bring:
  • + Master's degree in Physical Therapy from an accredited program. + Current PT license in the state of employment (or eligibility to obtain). + CPR certification required. + Strong communication, assessment, and problem-solving skills. + Ability to work independently in a variety of home environments. + Valid driver's license, auto insurance, and reliable transportation.
  • Preferred Experience (Not Required):
  • + 1+ year of experience in a hospital, home health, or outpatient rehab setting + In-home therapy delivery + Geriatric rehab, orthopedic or neurologic rehab + Post-acute care or chronic condition management + Working as part of a mobile or interdisciplinary care team
