Overview We are seeking a dedicated and experienced Home Health Physical Therapist to join our healthcare team in a part time or prn capacity. The ideal candidate will provide exceptional patient care within home and community settings, utilizing their expertise in physiotherapy, anatomy, and physiology to improve patient outcomes. This role offers the opportunity to work with diverse populations, including pediatrics, geriatrics, and orthopedic patients, across inpatient, outpatient, hospital, and clinic environments. The therapist will be responsible for developing and implementing individualized care plans, discharge planning, and coordinating with multidisciplinary teams to ensure comprehensive care delivery. Responsibilities Assess patient needs through thorough evaluations of physical function, mobility, and pain levels. Develop and implement personalized care plans tailored to each patient's condition and recovery goals. Provide physiotherapy interventions such as trigger point therapy, orthopedics treatment, and other modalities to promote healing and mobility. Manage discharge planning by coordinating with healthcare providers and families to ensure seamless transition of care. Document patient progress accurately using Electronic Medical Records (EMR) systems while maintaining compliance with healthcare regulations. Educate patients and caregivers on exercises, safety measures, and health maintenance strategies for ongoing recovery. Monitor patient progress regularly and adjust treatment plans as necessary to optimize outcomes. Collaborate with physicians, nurses, social workers, and other healthcare professionals within inpatient, outpatient, home health, or community settings. Maintain current knowledge of medical terminology, anatomy, physiology, and evidence-based practices relevant to physiotherapy. Participate in ongoing professional development activities to stay updated on new techniques such as trigger point therapy or specialized orthopedic interventions. Experience Proven experience in home health care or community-based physiotherapy preferred. Background working in acute care hospitals or outpatient clinics is highly desirable. Familiarity with EMR systems for documentation and patient management is required. Experience working with diverse patient populations including pediatrics and geriatrics is advantageous. Knowledge of discharge planning processes and care coordination across various healthcare settings is essential. Prior exposure to inpatient settings or hospital environments will be considered a strong asset. Certification or training in specialized areas such as orthopedics or trigger point therapy is a plus.
