Job Description
Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will transform patient recovery and enhance quality of life. In this vital role, you will assess, develop, and implement personalized treatment plans to promote mobility, reduce pain, and restore functional independence across a geriatric populations. This position is for an Assisted Living Facility (ALF) in Reed City with an additional facility in Big Rapids, if willing to help out at both is a plus but if only wanting to work in one facility that is fine as well. We are looking for a full time, part time or PRN PT. This position offers great flexibility inside and ALF with an in house therapy gym. Duties Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, and functional limitations. Develop and execute tailored treatment plans utilizing evidence-based physiotherapy techniques such as trigger point therapy and orthopedics. Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care and discharge planning. Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes in an ALF setting. Maintain accurate clinical documentation adhering to healthcare standards for medical terminology and electronic health records (EHR/EMR systems). Educate patients and their families on rehabilitation exercises, safety measures, and home care routines to support ongoing recovery. Support discharge planning by assessing patient needs and educating family and caregivers as appropriate. Skills Proven experience practicing in a physical therapy role within acute care, inpatient hospital, outpatient clinic, ALF, or home health environments. Strong knowledge of physiology, anatomy, neurology, orthopedics, pediatrics, and geriatrics to tailor effective treatment plans. Proficiency with electronic health records (EHR/EMR systems) management and adherence to clinical documentation standards. Excellent client communication skills to motivate patients and explain complex treatment concepts clearly. Ability to work collaboratively with multidisciplinary healthcare teams to develop comprehensive care plans. Experience with discharge planning processes and working with geriatric patients or those requiring community-based care.
