Job Description
Job Overview We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to improve patients' mobility, reduce pain, and enhance overall quality of life. Your expertise will support a diverse patient population, including geriatric, neurological, orthopedic, and outpatient rehabilitation cases. The ideal candidate is energetic, detail-oriented, and committed to delivering exceptional patient care through evidence-based practices. Duties Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans aligned with clinical standards. Implement therapeutic interventions such as manual therapy, trigger point therapy, therapeutic exercises, and mobility training to facilitate recovery. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to coordinate care and optimize treatment outcomes. Manage clinical documentation accurately within electronic health records (EHR) systems while adhering to medical terminology and physiotherapy documentation standards. Assist in discharge planning by evaluating patient progress and recommending appropriate home health or outpatient community care services. Provide education and guidance to patients and their families on exercises, injury prevention, and ongoing self-care strategies. Maintain compliance with hospital, inpatient, outpatient clinic, and outpatient rehabilitation center protocols during treatment sessions. Support patients across various settings including inpatient hospital units, outpatient clinics, home health environments, and community-based programs. Skills Proven experience practicing in a physical therapy role within acute care, inpatient hospital settings, outpatient clinics or home health environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics and related medical terminology. Expertise in developing effective treatment plans for diverse populations including geriatric patients and those requiring neurological or orthopedic rehabilitation. Familiarity with outpatient experience and outpatient rehabilitation center procedures for patient assessment and therapy delivery. Skilled in using electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination. Ability to perform discharge planning efficiently while managing multiple patient cases simultaneously. Excellent client communication skills to foster trust and motivate patients throughout their recovery process. Working knowledge of physiotherapy techniques such as trigger point therapy and other manual therapies.
