We are seeking a compassionate, motivated Full-Time Physical Therapist (PT) to provide skilled home health therapy services throughout the Southeast Michigan area , including communities in Oakland, Macomb, Wayne and Washtenaw Counties. If you enjoy one-on-one patient care, clinical autonomy, and seeing the real-life impact of your therapy, we would love to have you join our growing team. 💙 What You'll Do As a Home Health Physical Therapist, you'll work directly with patients in their homes to improve strength, mobility, balance, safety, and overall functional independence.
You will:
Complete comprehensive physical therapy evaluations and identify functional limitations and rehabilitation needs. Develop individualized plans of care, measurable goals, and appropriate treatment strategies. Provide skilled therapeutic exercise, strengthening, gait training, balance training, transfer training, and functional mobility interventions. Assess fall risk, home safety, and appropriate use of assistive devices. Educate patients and caregivers on safety, fall prevention, mobility techniques, and Home Exercise Programs (HEPs). Monitor patient response and progress throughout the episode of care. Complete reassessments and modify treatment plans when clinically appropriate. Coordinate care with physicians, caregivers, nursing, therapy disciplines, and other members of the interdisciplinary team. Maintain accurate, timely, and compliant clinical documentation. Provide services consistent with physician orders, the established plan of care, and applicable Medicare/CMS requirements.
