Net Health Responsibilities Evaluate residents and develop individualized physical therapy plans. Provide one-on-one treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention. Monitor residents' progress and modify treatment plans as needed. Complete discharge planning and provide recommendations for continued care. Educate residents and caregivers on exercises, mobility strategies, and equipment. Direct and supervise Physical Therapist Assistants as appropriate. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Minnesota Physical Therapist license. Graduate of a CAPTE-accredited Physical Therapy program. Current CPR certification. Strong communication, organization, and documentation skills. Geriatric, independent living, assisted living, memory care, skilled nursing, or rehabilitation experience is preferred. Why Join Oasis?
