Full Job Description Responsibilities/Job Description:
We are seeking a pediatric physical therapist to join our team at Fairview Masonic Children's University Hospital in Minneapolis! This PT will work in our outpatient clinic and have the opportunity to be part of a highly collaborative pediatric team that places a strong emphasis on evidence-based care and family involvement. The Fairview Masonic Children's University Hospital campus provides a wide range of medical specialties along with pediatric therapy and is designed to foster the highest level of collaboration between specialty physicians and pediatric therapists. Ideal candidate will have pediatric pelvic floor experience and experience with POTs patients. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

  • Friday; regular business hours with a few evenings, likely until 5:30pm or 6pm.

Responsibilities The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation Gathers pertinent data Performs examination Determines treatment diagnosis Develops an individualized plan of care through collaboration with the patient and family Analyzes data to identify needs and potential risks Determines prognosis for recovery Establishes individualized goals with patient and/or caregiver Individualizes plan of care considering developmental level and cultural differences Implements and modifies plan of care based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care Establishes appropriate discharge plan Partners with patients and families to provide skilled intervention Implements plan of care Alters treatment/plan of care to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education Required Qualifications Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota Basic Life Support (American Heart Association or Red Cross) Preferred Qualifications Doctorate of Physical Therapy 1 year experience Pediatric outpatient pelvic floor experience
