Job Description

Job Overview We are seeking a dedicated and skilled Physical Therapist (PT) to join Evolve Therapy Services at Shakopee Friendship Manor as an In House PRN. The ideal candidate will possess a strong background in physiotherapy, with experience in outpatient, inpatient, and home health settings. This role offers the opportunity to deliver high-quality patient care across diverse clinical environments, including acute care, geriatrics, pediatrics, neurology, orthopedics, and community-based care. The successful candidate will be committed to improving patient outcomes through evidence-based practices and comprehensive treatment plans while adhering to clinical documentation standards. Duties Assess patients' physical conditions and develop individualized care plans tailored to their specific needs and goals. Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote mobility and reduce pain. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings. Document patient progress accurately in electronic health records (EHR) systems in compliance with clinical documentation standards. Provide education to patients and their families regarding physiotherapy techniques, injury prevention, and health maintenance strategies. Manage caseloads efficiently within Skilled Nursing Facility. Stay current with advances in physiotherapy practices, physiology knowledge, and medical terminology to enhance patient care quality. Support the development of care plans that address complex conditions in geriatrics. Qualifications Valid licensure as a licensed Physical Therapist (PT) with practicing credentials in the relevant jurisdiction. Proven experience working with geriatric patients, pediatric populations, or neurological rehabilitation is highly desirable. Demonstrated expertise in discharge planning, acute care experience, and outpatient clinic settings. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards. Familiarity with electronic health records (EHR) management systems and EMR systems used within hospital or outpatient environments. Experience in outpatient rehabilitation centers or inpatient hospital settings is preferred. Ability to work independently while maintaining excellent patient care standards in a fast-paced environment. Excellent communication skills for effective collaboration with healthcare teams and engaging with patients. $50.00 - $58.00 per hour
