Home Health Care, Inc. is hiring a Physical Therapist (PT/DPT) to serve a defined northern Metro territory covering Bethel, Cedar, East Bethel, Oak Grove, and St. Francis . Be the Clinician Driving the Therapy Plan In home health, your evaluation matters. You're not simply working from a standard protocol. You're determining what this particular patient needs to safely function in their own environment. You'll evaluate mobility and functional limitations, establish the plan of care, determine appropriate interventions, monitor progress, make changes when necessary, and coordinate with PTAs and the broader clinical team. Your clinical reasoning sets the direction. And because you're treating patients where they actually live, you can see whether that plan is translating into meaningful function. Others may be dealing with repeated falls, Parkinson's disease, generalized weakness, balance deficits, or chronic conditions that have gradually made everyday activities harder. Your job is to determine what's limiting function and what needs to happen next.
You'll:
Perform comprehensive in-home Physical Therapy evaluations Develop individualized plans of care Establish functional goals based on each patient's needs Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility Assess fall risks and home safety concerns Recommend appropriate equipment and modifications Educate patients and caregivers Monitor outcomes and modify treatment plans based on progress Provide appropriate supervision and direction to PTAs Collaborate with physicians, nurses, OTs, SLPs, and other clinicians Complete accurate and timely clinical documentation Provide care throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis Your Experience Transfers We're interested in Physical Therapists from Home Health, Acute Care, Inpatient Rehabilitation, Outpatient Orthopedics, Skilled Nursing, Transitional Care, Senior Living, Long-Term Care, Hospital Rehab, and other therapy settings . Home health experience is valuable, but it isn't the only path here. If you can evaluate a patient, build an effective plan of care, communicate clearly, manage your schedule, and make confident clinical decisions, you already bring many of the skills this role requires.
What You Bring Required:
Active Minnesota Physical Therapist (PT) license Valid driver's license and reliable transportation Ability to provide patient care throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis Strong clinical assessment and decision-making skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule
Preferred:
Home health experience OASIS experience Geriatric, orthopedic, neurological, or post-acute experience Experience working with or supervising PTAs You Earned the PT License.
