We have Full-Time, Part-Time, and PRN opportunities available , giving you the flexibility to find the schedule that works best for you. $10,000 Sign-On Bonus for Full-Time Positions! Patient locations can vary throughout South City and Downtown St. Louis , so this position is a great fit for a PT who enjoys independence, flexibility, and working with patients in the community. What You'll Do Provide physical therapy services to patients in their homes Complete PT evaluations and develop individualized plans of care Establish treatment goals and discharge plans Provide education to patients, caregivers, and families Supervise and collaborate with Physical Therapist Assistants Communicate with physicians and the interdisciplinary care team Document visits accurately and in a timely manner Help patients improve mobility, safety, and independence in their home environment Participate in case conferences and team meetings Participate in a rotational weekend and holiday on-call schedule Why Home Health?
