Role:

As a Per Diem Physical Therapist (PT) , you will work part-time, as needed, to assess, plan, and implement interventions for our residents. Physical Therapist (PT) will also maintain accurate and up-to-date records of treatment plans, progress, and outcomes. Physical Therapist (PT)

Responsibilities:

Conduct thorough assessments of residents' physical abilities and develop individualized treatment plans Implement and adjust treatment plans to achieve desired outcomes, ensuring residents' safety and comfort Provide education and training to residents, family members, and caregivers on physical therapy techniques and exercises Collaborate with other members of the therapy team, including occupational and speech therapists, to ensure comprehensive care Maintain accurate and timely documentation of resident records, treatment plans, and progress Participate in quality improvement initiatives to continuously improve care and services
