Deliver evidence-based physiotherapy interventions including therapeutic exercises, manual therapy, trigger point therapy, and mobility training to promote patient rehabilitation. Manage discharge planning processes by coordinating care transitions and ensuring continuity of care across settings such as inpatient hospitals, outpatient clinics, and community environments. Document all patient interactions accurately within electronic health records (EHR) systems, adhering to medical terminology and clinical documentation standards. Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to optimize treatment outcomes and update care plans regularly. Educate patients and their families on treatment techniques, safety precautions, and home exercise programs to support ongoing recovery. Participate in outpatient rehabilitation center activities when needed, providing specialized services for orthopedic, neurological, pediatrics, or geriatrics cases. Skills Proven experience practicing in a physical therapy role within home health or outpatient settings with a strong understanding of physiology and anatomy. Knowledge of discharge planning procedures and working with geriatric patients across diverse environments such as hospitals, clinics, or community-based programs. Familiarity with clinical documentation standards and electronic health records (EHR) management for accurate patient care records. Ability to develop effective treatment plans tailored to individual patient needs while collaborating seamlessly with healthcare teams. Experience working with pediatric populations and understanding the nuances of pediatric physiotherapy is a plus. Strong communication skills for client engagement and education; capable of explaining complex medical terminology clearly. Competence in outpatient rehabilitation techniques including orthopedics, neurology, trigger point therapy, and inpatient care strategies.
