We currently serve patients throughout: St. Louis County Franklin County Parts of Jefferson County South County West County PRN coverage areas are based on patient needs, therapist availability, and current agency needs.

WHAT YOU'LL DO

As a Home Health Physical Therapist, you will: Evaluate patients in their homes and develop individualized treatment plans Provide skilled physical therapy services according to physician orders and the plan of care Help patients improve strength, mobility, balance, safety, and independence Provide therapeutic exercises and functional activities Educate patients, families, and caregivers Monitor patient progress and adjust treatment as needed Communicate with physicians and other members of the care team Participate in care coordination and case conferences Complete accurate and timely documentation

REQUIREMENTS

Current Missouri Physical Therapist (PT) license - Required Valid driver's license and reliable transportation Ability to travel to patients' homes Home health experience preferred but not required
