Job Description
Tender Touch Rehab Services seeks a compassionate Physical Therapist to provide patient-centered care for seniors in skilled nursing and long-term care settings. In this on-site role, you will perform comprehensive evaluations, develop individualized, evidence-based treatment plans, and deliver therapeutic interventions to improve function, independence, and quality of life. You will collaborate closely with an interdisciplinary team, maintain thorough EMR documentation, and educate patients, families, and staff. Responsibilities Evaluate senior patients in skilled nursing/long-term care using standardized assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercise, balance, gait, and functional mobility training Document evaluations, progress notes, and discharge summaries in EMR accurately and timely Collaborate with nursing, physicians, and interdisciplinary team to optimize outcomes Educate patients, families, and caregivers on safety, mobility, and home programs Participate in care conferences and contribute to discharge planning Support a compassionate, patient-centered environment that promotes dignity and independence Mentor and support therapy assistants and new team members as appropriate Required Skills Geriatric physical therapy Therapeutic exercise prescription Functional mobility and transfer training Balance and gait training Treatment planning and documentation (EMR) Knowledge of Medicare and reimbursement rules Interdisciplinary care coordination Patient and family education Fall risk assessment and prevention Wound care and pain management basics
