Job Description

Evaluates, plans, and implements treatment programs for the rehabilitation of patient, as directed by a physician, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation. > Additional Information

• Ability to communicate clearly to patients, fine motor skills required. Interpersonal Skills
• Displays a friendly, positive attitude and communicates with patients, families, physicians and co-workers in a professional manner. Ability to communicate both verbally and written. Essential Physical Requirements
• Must move, kneel, crouch, stoop, reach, stand, push, pull, and lift. Must follow good body mechanics. Essential Mental Abilities
• Assess, evaluate, treat, and educate patients. Must be adaptable to the many challenges of differing home environments. Also includes explaining, forecasting, organizational skills and time management. Essential Sensory Requirements
• Visual, hearing and feeling skills. Exposure to Hazards
• Frequent exposure to communicable diseases, toxic substances, body fluids, road and weather hazards. Other Skills and Abilities
• Must be registered and successfully pass background check with the Family Care Safety Registry and other applicable state registries.
