Job Description

Mercy is seeking a Per Diem Physical Therapist to join our outpatient team. In this role, you will assess, plan, and deliver individualized rehabilitation programs to improve mobility, function, and quality of life. Responsibilities include performing evaluations, developing treatment plans, documenting progress, and educating patients and families. You'll collaborate with an interdisciplinary care team while upholding Mercy's values of compassion, respect, and diversity, and enjoy opportunities for professional growth and continuous learning in a supportive environment. Responsibilities Perform comprehensive physical therapy evaluations for outpatient patients Develop and implement individualized treatment plans and goals Provide evidence-based therapeutic exercises and manual therapy Monitor, document, and report patient progress in the EMREducate patients and families on home exercise programs and injury prevention Collaborate with physicians, nurses, and other care team members Ensure compliance with regulatory, safety, and documentation standards Contribute to quality improvement and best-practice initiatives Participate in ongoing professional development and training Support Mercy's culture of compassion, diversity, and patient-centered care Required Skills Orthopedic physical therapy Neurologic rehabilitation Patient evaluation and assessment Treatment plan development Therapeutic exercise prescription Manual therapy techniques Use of rehab equipment and modalities Electronic medical record documentation Patient and family education Interdisciplinary team collaboration
