Mercy is seeking a compassionate Physical Therapist to provide evidence-based rehabilitative care for patients across acute and outpatient settings. In this full-time day shift role, you will evaluate functional limitations, develop individualized treatment plans, and deliver therapeutic interventions to restore mobility, reduce pain, and improve quality of life. Collaborating closely with physicians, nurses, and other therapists, you will document progress, educate patients and families, and support safe discharge planning while contributing to a culture of innovation, teamwork, and continuous learning at Mercy. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized treatment plans Deliver therapeutic interventions to improve strength, mobility, balance, and function Document assessments, goals, and patient progress accurately in the EMRCollaborate with physicians, nurses, and interdisciplinary teams on care plans Educate patients and families on home exercise programs, safety, and self-management Participate in discharge planning to ensure safe transitions of care Adhere to regulatory standards, ethical practice, and Mercy clinical protocols Contribute to quality improvement initiatives and evidence-based practice Supervise and mentor PTAs and students as appropriate Engage in continuing education and professional development Required Skills Physical therapy evaluation Therapeutic exercise Manual therapy Neuromuscular re-education Gait and balance training Modalities (e-stim, ultrasound, heat/cold) Treatment planning and documentation (EMR) Patient and family education Interdisciplinary care coordination Discharge and home exercise planning
