Job Description

The Physical Therapist provides physical therapy services to patients, staff and the community to prevent and treat disability following disease, injury or other cause. The therapist evaluates, plans, provides treatment and collaborates with others to optimize functional recovery and minimize disability. Evaluates, plans and provides care to optimize functional recovery of patient Collaborates patient / family and health care team to plan Provides patient care interventions based on patient acuity and functional status Alters the plan of care and updates goals based on patient�s response to treatment Participates in planning and operational changes to achieve team and individual performance improvement goals Observes and documents patient�s response to treatment and progress Assures accurate, concise and efficient documentation of patient progress in patient record Communicates patient care needs with interdisciplinary health care teams Facilitates patient education, discharge planning and functional maintenance with interdisciplinary staff and utilizing available resources Serves as a patient advocate in identifying functional potential and promoting the appropriate utilization of services
Education:
Bachelor's Degree Successful completion of an accredited physical therapy program
Experience:
0 1 None Clinical experience in a hospital setting
Skills:
Monitoring/assessing performance of individuals to make improvements or take corrective action Thinking critically by using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems Using computers and Microsoft Office programs
