5x8 Days Are you a dedicated Physical Therapist looking for a rewarding opportunity in home health care? Do you have a passion for providing personalized care and making a difference in patients' lives within their own homes? If so, we have an exciting opportunity for you in beautiful Missoula, MT. As a Physical Therapist in our home health department, you will be responsible for evaluating and implementing rehabilitation interventions in a home setting. You will manage and supervise patient care, educate patients and families, and collaborate with a multidisciplinary team to ensure comprehensive care. Your role will be critical in helping patients achieve their rehabilitation goals while maintaining a high standard of care. Evaluate and establish care plans to meet patients' needs in their home environment Coordinate care with other disciplines and seek orders for additional services Provide direct patient care based on established care plans and physician orders Apply evidence-based practice guidelines and modify treatment as appropriate Master the CMS required OASIS and collaborate with the Quality team Understand insurance requirements in Home Health Perform interventions allowed under the physical therapy practice act, including wound care Participate in multidisciplinary conferences and communicate with physicians and staff Document all skilled services, adverse events, and communications in the medical record Engage in Quality Assurance Performance Improvement initiatives Manage productivity standards and maintain accurate time logs and mileage Oversee services provided by the Physical Therapist Assistant and supervise CNA when needed Plan admissions, OASIS time points, and discharges throughout the patient's care episode Maintain physical therapy licensure and participate in educational opportunities Perform other duties as assigned by supervisor Need 2 years experience in setting Apply today and let Capstone Health help you reach your full potential!
