As a valued member of our interdisciplinary team, you'll provide personalized, evidence-based care in a supportive skilled nursing environment that prioritizes both resident outcomes and professional growth. Position Summary We are seeking a licensed and motivated Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans for residents recovering from illness, injury, or surgery. This PRN position offers flexible scheduling and the opportunity to make a meaningful impact while working alongside a collaborative rehabilitation team. Key Responsibilities Perform comprehensive resident evaluations and develop individualized treatment plans based on physician referrals. Provide skilled physical therapy services designed to improve strength, balance, mobility, endurance, and functional independence. Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes. Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and therapy students as needed. Maintain accurate, timely documentation in compliance with facility policies and regulatory requirements. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate resident care. Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, and home exercise programs. Participate in discharge planning and quality improvement initiatives. Travel to nearby affiliated facilities as needed to support resident care. Qualifications Graduate of an accredited Physical Therapy program. Current North Carolina Physical Therapist license (or eligibility to obtain licensure). Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required. Strong clinical reasoning, communication, and interpersonal skills. Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.
