Collective Therapy is a locally owned, therapist-led physical therapy company providing outpatient care directly in patients' homes throughout South Charlotte. These are not homebound patients. Our patients choose mobile outpatient therapy because they value the convenience and the opportunity to work on mobility, strength, balance, safety, and function in their own environment. Your responsibilities will include:

Performing physical therapy evaluations and treatments Developing individualized plans of care Providing strength, balance, mobility, and functional interventions Addressing fall prevention and safety Adapting treatment to each patient's home environment Monitoring progress and updating treatment plans Communicating with patients, families, and the interdisciplinary care team Completing timely and accurate documentation Building meaningful, long-term relationships with patients What We're Looking For We're looking for a Physical Therapist who is compassionate, dependable, clinically confident, and comfortable working independently.

Required:

Active North Carolina Physical Therapist (PT) license Strong clinical judgment Excellent communication and organizational skills Dependable and patient-focused Preferred, but not required: Home health experience Geriatric experience Experience treating adult and older adult populations New graduates are welcome to apply.
