Schedule & Setting:

Full-time, 13-week contract Monday to Friday, 8:00 AM 5:00 PM Expectation of 2532 points per week (typically 57 patient visits per day) Field-based, providing care directly in patient homes Daily travel within your assigned territory (mileage reimbursement provided) Electronic documentation using Home Care Home Base (HCHB) OASIS assessments for start of care, resumption, recertification, and discharge

Qualifications:

Active Physical Therapist license in good standing within the state of practice Minimum 1 year of recent home health physical therapy experience required Proficiency with Home Care Home Base (HCHB) preferred Prior experience completing OASIS assessments required Strong clinical skills in functional assessment and care planning Reliable transportation, valid drivers license, and current auto insurance Ability to work autonomously and manage a flexible field-based schedule Exceptional communication, time management, and organizational skills

Responsibilities:

Complete comprehensive physical therapy evaluations and OASIS assessments Develop and implement tailored plans of care for mobility, safety, and functional independence Deliver evidence-based therapeutic interventions and monitor patient progress Educate patients and caregivers on home exercise programs and safety techniques Accurately document visits in HCHB and coordinate with the interdisciplinary care team
