Responsibilities Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, and functional abilities. Develop and implement customized treatment plans utilizing techniques such as manual therapy, trigger point therapy, and therapeutic exercises. Monitor patient progress regularly, adjusting care plans to optimize recovery outcomes. Collaborate effectively with healthcare professionals including physicians, nurses, and occupational therapists to coordinate care strategies. Manage clinical documentation standards accurately within electronic health records (EHR) and electronic medical records (EMR) systems. Assist with discharge planning by preparing patients for transition to home or community care environments. Provide education to patients and their families on injury prevention, rehabilitation exercises, and health maintenance strategies. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Demonstrated expertise in outpatient rehabilitation center environments with a focus on orthopedics, neurology, geriatrics, pediatrics, or sports medicine. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards necessary for effective treatment planning. Experience working with diverse patient populations including geriatric patients and those requiring post-surgical or neurological rehabilitation. Familiarity with outpatient experience and inpatient hospital experience is highly desirable. Proficiency in managing outpatient care plans involving discharge planning and working with electronic health records (EHR) systems. Ability to communicate clearly with clients about treatment goals and progress while collaborating with multidisciplinary healthcare teams to deliver comprehensive patient care.
