long-term care / residential care Responsibilities Conduct initial physical therapy evaluations and ongoing re-evaluations for residents in a long-term care setting. Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence. Provide therapeutic interventions (therapeutic exercises, neuromuscular re-education, and appropriate modalities) to address identified deficits. Monitor progress, document sessions in the patient record, and adjust goals as residents progress. Collaborate with an interdisciplinary team (nursing, occupational therapy, speech-language pathology, social work) to optimize care and discharge planning. Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification.
