Long-term care facility rehabilitation Responsibilities Conduct initial evaluations and ongoing reassessments to determine resident needs and progress. Develop and implement individualized plans of care for residents, focusing on functional mobility and independence. Provide effective physical therapy interventions, including therapeutic exercise, gait and balance training, and mobility training as appropriate. Monitor resident progress, document treatment sessions in the medical record, and communicate with the interdisciplinary care team. Educate residents and families on home exercise programs, safety, fall prevention, and discharge planning. Ensure safety during therapy sessions, adhere to infection control protocols, and maintain accurate, compliant documentation.
