Accepted We are seeking an experienced Physical Therapist (PT) to provide home health physical therapy services in Grand Island, Nebraska. The Physical Therapist will evaluate patients in their home environment, develop individualized treatment plans, provide therapeutic interventions, and help patients improve mobility, strength, balance, and functional independence. This is a 13-week contract opportunity with a full-time schedule of approximately 40 hours per week . Responsibilities Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized PT treatment plans based on patient needs and functional goals. Provide home health physical therapy interventions to improve mobility, strength, balance, coordination, and range of motion. Assist patients with functional mobility, transfers, gait training, and therapeutic exercises . Monitor patient progress and modify treatment plans as needed. Educate patients and caregivers regarding home exercise programs, safety, mobility, and injury prevention. Document patient evaluations, treatment sessions, progress, and outcomes accurately. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the home health care team . Provide patient-centered care while maintaining professional and clinical standards. Requirements Current Physical Therapist (PT) license/eligibility to practice in Nebraska. BLS Certification required. Previous Physical Therapy / Home Health experience preferred. Strong clinical assessment, communication, documentation, and patient-care skills. Ability to work independently in a home-based care environment. Ability to work 40 hours per week . Must be able to work Monday-Friday , or a shifted schedule such as Tuesday-Saturday or Sunday-Thursday . Day-lengths may vary slightly depending on the schedule and patient needs.
