Job Description

Physical Therapist (PT) Part-Time / PRN Brookestone Home Health Omaha Metro Area Make a Difference Where It Matters Most Brookestone Home Health is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a Part-Time or PRN basis . This position provides the opportunity to deliver one-on-one care to patients in their homes throughout the Omaha metro area, helping them regain independence, improve mobility, and achieve their highest level of function. We are looking for a clinician who values patient-centered care, flexibility, and the ability to make a meaningful impact in the lives of patients and their families. Position Summary The Physical Therapist is responsible for evaluating and treating patients in their homes, developing individualized treatment plans, and collaborating with the interdisciplinary care team to achieve optimal patient outcomes. Essential Functions Conduct physical therapy evaluations and reassessments. Develop and implement individualized treatment plans. Provide therapeutic exercise, gait training, balance training, transfer training, and fall prevention education. Assess the need for assistive devices and durable medical equipment. Educate patients and caregivers on treatment plans and home exercise programs. Monitor and document patient progress toward established goals. Collaborate with physicians, nurses, and other healthcare professionals regarding patient care. Complete clinical documentation accurately and in a timely manner. Maintain compliance with agency policies, professional standards, and regulatory requirements.
Qualifications Required:
Graduate of an accredited Physical Therapy program. Current Nebraska Physical Therapist license in good standing. Current CPR certification. Valid driver's license, reliable transportation, and proof of automobile insurance. Ability to travel throughout the Omaha service area.
Preferred:
Home health experience. Experience working with geriatric and rehabilitation populations. Knowledge of Medicare and home health documentation requirements.
