Franciscan Healthcare is looking for a Physical Therapist to carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. This position will provide services in the following settings: pediatrics, outpatient, inpatient, nursing home, home health and local schools.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

Plan, refer, screen, evaluate, carry out, and discharge patient's treatment program in coordination with other medical professional involved in the patients care. Administer treatment program per physician instructions. Offer effective communication to patients regarding treatment plan, education on condition, necessary precautions and discharge goals. Provide quality services to all patient we treat. Complete documentation within the respective EMR based on the entity which treatment was performed.

QUALIFICATIONS

Education and/or Experience Graduated from an accredited School of Physical Therapy Licensed Physical Therapist in Nebraska Valid driver's license Knowledgeable of EMR systems (Cerner, OptimisPT, NetHealth and Matrix Care). If previously experience is not had, capable of learning functions within each system Other Competencies Ability to use critical thinking skills to organize workload according to priorities and assess situations Ability to maintain confidentiality of sensitive information Ability to record information accurately and follow instructions precisely Interpersonal skills, specifically courtesy and tact in order to communicate effectively Ability to walk and stand for long periods of time; May be on feet for most of the shift Occasionally lift and carry items weighing up to 50lbs; if weight to be lifted exceeds 35 lbs., assistive devices should be used
