Overview We are seeking a compassionate and collaborative Early Intervention Therapist (Physical Therapist, Physical Therapist Assistant) to join our Early Supports & Services (ESS) team. This role supports infants and toddlers (birth-3) with developmental delays and disabilities, working closely with families in natural environments such as homes and community settings. Per Diem | Part-time | Per-visit | Per-Evaluation Key Responsibilities Provide developmental assessments and ongoing evaluation of children ages 0-3 Deliver evidence-based therapy services aligned with each child's Individualized Family Service Plan (IFSP) Coach and support caregivers using a family-centered, routines-based approach Collaborate with a multidisciplinary team (service coordinators, educators, therapists) Participate in IFSP meetings and contribute to goal development and progress monitoring Maintain timely and accurate documentation in compliance with ESS and state guidelines Provide services in home and community-based settings Qualifications Current licensure in New Hampshire as one of the following: Physical Therapist (PT), or Speech-Language Pathologist (SLP) Experience in pediatrics or early intervention preferred Strong communication and family engagement skills Ability to work independently and as part of a team Valid driver's license and reliable transportation Knowledge of early childhood development and coaching models Experience with IFSPs and natural environment service delivery Why Join an ESS Program?
