•60/hourWe are seeking a PRN Physical Therapist (PT) to support our rehabilitation team. This role offers flexible scheduling while providing the opportunity to help residents maximize mobility, strength, and independence in a skilled nursing and post-acute care setting.

Position Responsibilities:

Complete physical therapy evaluations and develop individualized treatment plansProvide treatment focused on gait, balance, strengthening, transfers, mobility, endurance, and pain managementMonitor resident progress and modify treatment plans as appropriateCollaborate with PTAs, OTs, SLPs, nursing staff, and physicians to coordinate resident careEducate residents and caregivers on exercises and safety techniquesComplete accurate and timely documentation
