Your Role and Responsibilities:

As a Physical Therapist, you will:

Assess and Evaluate :

Conduct thorough evaluations of patients' muscle strength, mobility, gait, range of motion, and potential for rehabilitation.

Develop Care Plans :

Create personalized treatment plans in coordination with physicians and the care team, tailored to the needs of each patient.

Deliver Therapy :

Provide direct physical therapy treatments and guide patients in the use of prosthetic devices and therapeutic appliances.

Instruct and Educate :

Teach patients and their families/caregivers about exercises, treatment goals, and therapeutic techniques.

Support Home Care :

Offer specialized care and consultation for patients in their home environments, helping them regain function and independence.

Document and Report :

Maintain accurate clinical records, report patient progress, and participate in discharge planning.

Supervise and Coordinate :

Supervise physical therapy assistants and collaborate with other team members to ensure the highest quality of care.

What We're Looking For:

We seek a Physical Therapist who is passionate about improving patients' quality of life and has the following qualifications: A degree in physical therapy from an accredited program. A valid license to practice as a Physical Therapist in the state. At least two years of clinical experience as a physical therapist, with home health or hospice experience preferred. Strong communication, organizational, and documentation skills. Current CPR certification. A valid driver's license and access to a reliable, insured vehicle. Why Join Us? $45.00 - $50.00 per hour
