We are seeking licensed Physical Therapists to provide individualized, one-on-one care to older adults in private homes and senior living communities, utilizing our Geriatric House Calls™ model.
Responsibilities include:
Conducting evaluations and developing personalized treatment plans Delivering evidence-based therapy sessions to enhance mobility, independence, and quality of life Documenting care accurately within electronic medical records Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists Educating patients, caregivers, and staff on safety and aging-in-place strategies Providing telehealth and virtual care to increase access and flexibility Qualifications and skills: Valid Physical Therapy license or eligibility in practice state(s) Degree from an accredited Physical Therapy program Proficiency with computers and EMR systems Current CPR certification Physical requirements: Ability to exert moderate physical effort, including lifting, repositioning patients, and assisting with transfers Capability to perform CPR and navigate patient environments Willingness to drive to patient locations and wear PPE as needed This role offers autonomy, flexible scheduling, and opportunities for professional growth in a supportive, interdisciplinary environment.
