You will:

Deliver Personalized Care:

Perform meaningful home visits to clients across Hudson or Essex County, transforming their living space into a center of rehabilitation.

Assess & Innovate:

Perform diagnostic tests and physical measurements to develop and implement highly individualized care plans.

Monitor & Adapt:

Perform continual assessments of patient progress and update plans of care to match their evolving needs.

Educate & Empower:

Provide vital education and hands-on instruction to patients, their caretakers, and family members to ensure safe, long-term recovery.

Collaborate Dynamically:
