Job Description

Physical Therapist Position Summary Provides medically prescribed clinical physical therapy in patients' homes. Primary Position Responsibilities 1. Facilitate and Coordinate Physical Therapy Care. 2. Evaluates Progress Toward Attainment of Patient Outcomes. 3. Documentation of Care. 4. Employs Strategies to Promote a Safe Environment and Patient Health. 5. Education of the Patient and Care Giver. 6. Other duties as assigned. Work Contact Group (Internal/External) HMC employees, health care providers, physicians, non-physician providers, community resources, patients, families, significant others. Reporting Relationships Reports to (position): Clinical Coordinator of Therapy Services Supervises (position(s):
None Qualifications Minimum Education:
Required:
Bachelor's Degree in
Physical Therapy Preferred:
Master's or Doctoral Degree in Physical Therapy Minimum Years of Experience (Amount, Type and Variation):
Required:
Two years of clinical experience in a hospital, out-patient, or rehab setting.
Preferred:
One year of Home Health experience.
License, Registry or Certification :
Required:
  • Current license to practice Physical Therapy in the State of New Jersey.
  • Valid driver's license.
  • Current CPR certification.
If not certified, must obtain certification within six (6) months of hire Knowledge, Skills and/or
Abilities :
Required:
  • Excellent communication, time management, judgment and decision making skills.
  • Computer functionality knowledge.
  • Knowledge and skill in current PT practice.
Preferred:
