Job Description

Position OverviewWe are seeking a highly skilled and compassionate Physical Therapist to join Parkland Physical Therapy Clinic, based in Freehold, New Jersey.

This full-time position operates in a home care setting, allowing you to provide personalized therapy to patients in the comfort of their own homes.

The ideal candidate will possess a Doctorate in Physical Therapy (DPT) and demonstrate a commitment to enhancing patient outcomes through evidence-based practices and tailored rehabilitation plans.

Annual compensation ranges from
USD 100,000
to 120,000, commensurate with experience.

Work EnvironmentIn this role, you will primarily work onsite, visiting patients in their homes across Freehold and surrounding areas.

Key ResponsibilitiesConduct comprehensive evaluations that include patient history, specific assessments, and the formulation of individualized treatment plans, ensuring meticulous documentation of results.

Effectively communicate evaluation findings and treatment strategies to patients and their families, actively involving them in goal-setting to foster ownership of their rehabilitation journey.

Implement tailored treatment protocols based on assessment outcomes, adapting interventions as necessary to align with patients' evolving needs and healthcare trends.

Collaborate with interdisciplinary healthcare professionals to provide holistic care; recognize and address changes in patient conditions that necessitate modifications to the care plan.

Manage discharge planning and follow-up processes, ensuring a comprehensive understanding of patient care pathways and maintaining accurate and timely documentation of all patient interactions and treatment sessions.

Demonstrate effective time management skills by balancing patient caseloads, adhering to a structured schedule, and participating in marketing initiatives to promote our services within the community.

Contribute to the department's objectives by engaging in compliance with healthcare regulations and enhancing service delivery standards.

Commit to professional development by pursuing continuing education opportunities and serving as a clinical instructor to students and new staff.

Ensure a positive patient experience by being adaptable and responsive to individual patient needs while actively participating in our Access NRH initiatives to improve community engagement.

Patient Population / Client FocusThe ideal candidate will work with a diverse range of patients, including those recovering from surgery, trauma, stroke, and chronic conditions.

Our patient-centered approach ensures that therapies are individualized, taking into account each patient's unique circumstances and rehabilitation goals, which can lead to meaningful improvements in their quality of life.

QualificationsA Doctorate degree in Physical Therapy (DPT) is required.

A valid DPT license in the State of New Jersey is essential.

Current American Heart Association's BLS for Healthcare Providers CPR certification is necessary.

State-specific licensure may be required depending on the geographical locations served.

Preferred ExperienceWhile prior clinical experience is not mandatory, a background in home health care settings or previous internships during academic training will be advantageous.

Experience in developing treatment plans for a varied patient population is beneficial.
