Implement evidence-based physiotherapy interventions to facilitate recovery in outpatient clinics, inpatient units, hospitals, or home health environments. Monitor patient progress regularly, adjusting care plans as needed to optimize rehabilitation outcomes. Document all clinical activities accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure comprehensive patient care. Educate patients and their families on treatment procedures, post-discharge care plans, and community resources for ongoing support. Participate in discharge planning processes to facilitate smooth transitions from hospital or inpatient settings to home or community care. Qualifications Valid licensure as a Physical Therapist with practicing credentials in the relevant jurisdiction. Proven experience in acute care, outpatient clinics, inpatient hospital settings, or home health environments. Strong knowledge of physiology, anatomy, neurology, orthopedics, and geriatrics to inform treatment strategies. Familiarity with clinical documentation standards and electronic health record (EHR) management systems such as EMR platforms. Experience working with diverse patient populations including pediatrics, geriatrics, and neurological cases. Excellent communication skills for client interaction and interdisciplinary collaboration.
