Full Job Description Role:
Per Diem Orthopedic Home Physical Therapist Responsibilities:
Evaluate and treat patients in their homes, focusing on orthopedic rehabilitation, including post-operative care for hip and knee replacements Educate patients on safe mobility, transfers, and ambulation, ensuring safety for discharge Develop and modify individualized treatment plans based on evaluations and goals Collaborate with interdisciplinary teams and support systems Document care, progress, and discharge plans accurately following regulatory and departmental guidelines Perform functional training, including ADLs/IADLs, therapeutic activities, and community reintegration Ensure safe, effective interventions and maintain compliance with policies Utilize EMR software for documentation and billing Participate in professional development activities such as billing and coding seminarsQualifications & Skills:
Licensed Physical Therapist with a Master's or higher degree; Doctorate preferred Strong orthopedic background, especially in total hip and knee replacements Excellent communication, clinical decision-making, and computer skills Ability to work independently and as part of a team with professionalism and dedication Experience with EMR software and understanding of reimbursement systems is preferredKey Traits:
