Work Environment This position is primarily onsite, with Physical Therapists providing care within patients' homes across Lakewood and the surrounding areas. Our work environment is dynamic and hands-on, requiring PTs to have adaptability and creativity in delivering therapies that fit within the client's home setting. Interactions with patients, their families, and interdisciplinary colleagues will happen regularly, fostering a collaborative atmosphere that enhances patient care. Key Responsibilities Conduct thorough assessments of patients' physical capabilities and establish individualized treatment plans designed to improve mobility, strength, and overall function. Guide patients through therapeutic exercises, stretches, and activities specifically tailored to their rehabilitation goals. Provide education on body mechanics, exercise techniques, and self-management strategies, empowering patients to take an active role in their recovery. Administer therapeutic modalities including ultrasound, electrical stimulation, and heat/cold therapy, ensuring safe and effective application under appropriate guidelines. Observe, document, and analyze patient progress meticulously, adjusting treatment plans as necessary to optimize outcomes. Maintain an organized and clean work environment, preparing treatment spaces and ensuring equipment is ready and functional for therapy sessions. Work collaboratively with Occupational Therapists, Speech-Language Pathologists, and other healthcare professionals to provide integrated patient care. Offer emotional support and encouragement to patients throughout their recovery, helping them stay motivated and focused on achieving their goals. Uphold ethical and professional standards, ensuring patient confidentiality and respect for their rights at all times. Patient Population / Client Focus The Physical Therapist at CareNow Cardio will work with a diverse array of patients, including individuals recovering from surgery, seniors aiming to maintain mobility, young athletes recovering from sports injuries, and those with chronic pain or neurological conditions. Each patient presents unique challenges and opportunities for growth, requiring the PT to be skilled, empathetic, and responsive to their needs. Qualifications Bachelor's Degree in Physical Therapy from an accredited institution. Valid state license or certification to practice as a Physical Therapist in New Jersey. Strong knowledge of human anatomy, physiology, and the latest therapeutic interventions. Excellent communication skills to effectively convey complex rehabilitation concepts to patients and their families. Demonstrated ability to work both independently and as part of a multidisciplinary team. Preferred Experience A minimum of one year of experience as a Physical Therapist, particularly in home care or outpatient settings, is highly desirable. Experience with diverse patient populations and varying rehabilitation needs will be advantageous. Familiarity with electronic health record (EHR) systems and documentation practices. Compensation and Benefits This position offers a competitive annual salary ranging between USD 100,000 and 120,000, commensurate with experience.
