Licensed Physical Therapist:
Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders and resident goals. Provide skilled physical therapy treatments designed to improve mobility, strength, balance, function, and overall quality of life. Monitor resident progress, modify treatment plans as appropriate, and accurately document all evaluations, treatments, and outcomes. Communicate resident progress, needs, and recommendations with residents, families, physicians, nursing staff, and other members of the interdisciplinary team. Participate in care planning meetings, facility committees, and community education initiatives as appropriate. Maintain professional standards of practice while protecting resident rights and confidentiality. Participate in continuing education and professional development to maintain clinical competency and stay current with best practices. Qualifications forLicensed Physical Therapist:
Graduate of an accredited Physical Therapy program required. Current, active, and unencumbered Physical Therapist license in the state of New Jersey required. Evidence of a current satisfactory physical examination and/or freedom from communicable diseases as required by state regulations. Strong clinical assessment, communication, and documentation skills. Ability to collaborate effectively with interdisciplinary healthcare teams.
