Work Environment This role operates onsite in patients' homes, providing a unique opportunity to deliver individualized care in a comfortable and familiar setting. Working Monday to Friday, our therapists delight in the flexibility of choosing their hours to support a healthy work-life balance. The home care aspect allows for hands-on engagement with patients in a non-clinical environment, which can foster deeper connections and enhance recovery outcomes. Key Responsibilities Conduct initial evaluations and ongoing assessments to determine the physical therapy needs of home care patients. Develop comprehensive treatment plans utilizing an array of therapeutic techniques such as manual therapy, therapeutic exercises, and modalities aimed at reducing pain and improving mobility. Implement individualized treatment strategies based on each patient's specific goals and progress, modifying therapy approaches as necessary. Document treatment sessions, patient responses, and progress meticulously to maintain accurate records and communicate effectively with other healthcare providers. Educate patients and their families on rehabilitation processes, promoting adherence to therapy recommendations and efficient use of assistive devices. Actively engage with caregivers and family members to provide training on home exercise programs and other interventions that can support the patient's recovery journey. Participate in continuous professional development through workshops, peer collaboration, and contributions to the physical therapy community to stay informed on evolving industry practices and tools. Patient Population / Client Focus Our patients include a wide range of demographics—children, adults, and seniors—who require physical therapy due to conditions stemming from surgery, sports injuries, neurological issues, or chronic pain. The goal is to provide evidence-based treatment that fosters independence and improves the quality of life, facilitating functional mobility and achieving personalized therapeutic milestones. Qualifications Bachelor's Degree in Physical Therapy from an accredited institution. A valid state license to practice as a Physical Therapist in New Jersey. Strong verbal and written communication skills to interact effectively with patients and their families. Proficient in using basic computer systems for documentation and patient tracking. A highly flexible mindset and adaptability to meet the dynamic demands of home care. A friendly and approachable demeanor, promoting a trustworthy therapist-patient relationship. Compensation and Benefits We offer a competitive salary ranging from $100,000 to $120,000 annually based on experience and expertise.
