Key Responsibilities As a Physical Therapist at Encompass Rehabilitation Center, your primary responsibilities will include: Developing comprehensive treatment plans tailored to each client's needs, with a focus on pain relief, muscle restoration, and improving mobility. Conducting thorough evaluations of clients' physical abilities utilizing advanced diagnostic tools to assess neuromuscular and functional capabilities. Supervising and mentoring physical therapy assistants in the implementation of individualized care plans to ensure high-quality treatment delivery. Documenting patient progress meticulously, including OASIS assessments, care plans, progress notes, and discharge summaries in accordance with regulatory standards. Adapting treatment strategies based on ongoing evaluations and communicating necessary changes to medical and interdisciplinary team members. Educating clients and families about rehabilitation programs and instructing them on the effective use and maintenance of therapeutic devices. Participating in case conferences with other healthcare professionals to coordinate comprehensive care and achieve positive patient outcomes. Patient Population / Client Focus Your expertise will serve a diverse range of clients, primarily focusing on individuals recovering from surgeries, injuries, chronic conditions, and age-related challenges. Our goal is to enable our clients to regain independence and mobility with tailored rehabilitation approaches designed to meet their specific needs. Qualifications To be successful in this role, candidates must meet the following qualifications: A Bachelor's degree in Physical Therapy; Doctorate degree in Physical Therapy (DPT) preferred. A valid physical therapy license in good standing within the State of New Jersey. At least two years of experience providing physical therapy in a clinical or home care environment. Current CPR certification and a valid driver's license with automobile insurance. A commitment to clinical excellence, integrity, and a collaborative approach to care. Exceptional interpersonal communication skills and the ability to engage effectively with patients and their families. A proactive mindset and the ability to make informed decisions independently while contributing to a positive team environment.
