The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.Job Overview:
The physical therapist provides physical therapy services in accordance with New Jersey Practice Act, standards of accreditation and States regulatory bodies, and the Code of Ethics of the APTA. This includes examination, evaluation, and treatment of individuals for prevention of injury and/or rehabilitation of physical disability, functional deficit, and pain. The physical therapist implements physical therapy treatment in coordination with the multidisciplinary team.Qualifications:

Develop an individualized treatment program based on the evaluation findings focusing on time-oriented, functional, and measurable short and long-term goals. Complete patient evaluation within the specified period; ensure it contains data that is reflective of a complete, holistic assessment of the client's condition. Prepare appropriate and precise documentation, such as progress notes, reassessments, and discharge summaries according to policy and procedures; submit in a timely manner to support the need for continued skilled physical therapy. Act as liaison between the physician, case manager, and other members of the health care team to ensure effective coordination, and appropriate resource referral. Ensure service delivery follows the frequency and duration of the physician's plan of care for the patient. Provide comprehensive education to patients and/or caregivers based upon their identified needs in a manner that they are able to understand. Create a rapport with the patient to foster an optimum environment for goal achievement. Document case communication with other members of the health care team as appropriate throughout the patient's course of care utilizing secure messaging through an appropriate application. Complete scheduling requirements timely and accurately by updating and submitting new and changed schedules through appropriate system applications. Implement the treatment plan utilizing correct and appropriate clinical skills and techniques. Participate in discharge planning throughout the patient's course of care; address problem areas appropriately. Confirm discharge summary indicates the patient's status, outcomes achieved, and recommendations for further services or interventions. Independently meet productivity requirements on a consistent basis. Demonstrate flexibility with job responsibilities in all areas.Benefits and Perks:
